ASC Accomplishments

Two ASC workgroups recently launched projects designed to address the needs of Santa Clara County older adults and their caregivers.

• The Caregiver Support workgroup published. printed, and is now distributing a brochure. The brochure will enable caregivers to easily identify and access resources available to them in Santa Clara County. To preveiw a PDF of the caregiver brochure in English click here (250 Kb). The caregiver borchure is also conveniently available in Spanish (254 Kb), Vietnamese (325 Kb), and Chinese (340 Kb).
• The Health and Wellness workgroup secured funding to begin a case management program that will facilitate better Hospital-to-Home transitions and reduce patient need for re-hospitalization.
